WELCOME TO THE NISKAYUNA HIGH SCHOOL NURSES’S OFFICE
The Nurse’s Office is open daily from 7:30 am to 4:00 pm during the regular school year.
The nursing staff is available to provide medical assistance to students who may become ill or injured during the school day. If a student needs to go home, a parent/guardian will be notified and the parent will arrange for transportation. Students are NOT permitted to be dismissed with another student.
All parents are required to complete the Confidential Health Office Emergency Card handed out at the beginning of the year. This provides the nursing staff with emergency contacts and telephone numbers. This information is necessary should an emergency situation develop.
MEDICATION IN SCHOOL
Students are not allowed to carry medication in school, with the exception of rescue medication (Epi-pens and Inhalers). In order for the school nurse to administer medication, either prescription or over the counter, the following are required by the State of New York:
- An order from a licensed prescriber including name of medication, dosage and time to be administered
- Parent/Guardian permission to administer the medication
- Medication must be delivered in the ORIGINAL properly labeled container

PHYSICALS
New York State mandates physicals for all students in Grade 10. You may have this done by your own health care provider. Completed Medical Inspection Forms are due to the Nurse’s Office December 1st . If you prefer to have your child examined by the school physician, physicals are scheduled for early December. If we have not heard from you by December 1st, we will schedule your child to be seen by our school physician, Dr. Albert Loffredo. A sports physical is acceptable as a Tenth Grade physical.

WORKING PAPERS
Students may obtain their 14-15 year old or 16-17 year old working papers in the Nurse’s
Office. A parent completed application is required and students must have a valid physical dated within one year of the application. During the summer vacation, working papers will be issued in the Attendance Office.

FIRST AID
Basic first aid is rendered to all who may require it. If the injury is extensive, a paramedic call may be made to transport the student.
Accident/Incident reports are made on all serious injuries. The school district has student insurance coverage for any outstanding bills your personal insurance company does not cover. If you require a form, please contact our office.
RESTRICTION OF PHYSICAL ACTIVITY
If your child has had an illness or injury that will prevent or limit them from participation in physical education classes, a signed note from your health care provider is required. Students cannot be excused from class with a written note from the parent/guardian.
ATHLETIC PARTICIPATION
Before any student can try out or participate in any sport, the student must be cleared through the NURSE”S OFFICE. There are no exceptions. In order to participate the student is required to submit the following:
1. Proof of a valid physical, dated within one year of the month of the start of the current sport season. Students need to check with the nurse to see if their physical is valid for that season.
2. A completed gray Athletic Permission Card. - All information must be filled in and signed by the parent/guardian. If information or valid signatures are omitted, the nurse will not sign the card. Only the school nurse may sign this card, NOT your physician.
3. A completed Interim Health Form
A NEW GRAY CARD AND INTERIM HEALTH FORM ARE REQUIRED FOR EACH SPORT SEASON
All completed paperwork must be submitted at least one week prior to the start of the season.Failure to hand in paperwork in a timely manner may result in the student not being able to try out for his/her sport.

SPORTS PHYSICALS
Sports physicals may be done by your health care provider or the school physician. School physicals for Fall Sports are offered in May/June and several dates in August.
Winter Sport physicals are offered in October and Spring Sport Physicals are offered in February. The school physicals are offered to the student at no charge.
